To access the National BQA Database, you will need to use the information you provided on your certification form when you completed your BQA certification.
If your first ever certification was before January 1, 2021, and this is your first time logging into the National BQA Database, your username is the first three letters of your first name, your last name, followed by the state abbreviation where you were certified (no spaces) and your password is BQAway.
Example: John Smith who certified in Nebraska
Username: johsmithne
Password: BQAway
If you certified after January 1, 2021, and this is your first time logging into the National BQA Database, your username is your phone number without hyphens and your password is BQAway.
Example: If your phone number is 308-555-1234, you would use the following credentials to login to the National BQA Database.
Username: 3085551234
